Ultrasound method may supplant biopsies

CHICAGO – An experimental ultrasound technique that measures how easily breast lumps compress and bounce back could enable doctors to determine instantly whether a woman has cancer or not — without having to do a biopsy.

In a small study of 80 women, the technique, called “elastography,” distinguished harmless lumps from malignant ones with nearly 100 percent accuracy.

If the results hold up in a larger study, elastography could save thousands of women from the waiting, cost, discomfort and anxiety of a biopsy, in which cells are removed from the breast — sometimes with a needle, sometimes with a scalpel — and examined under a microscope.

“There’s a lot of anxiety, a lot of stress, a lot of fear involved” with biopsies, said Susan Brown, manager of health education for the Susan G. Komen Breast Cancer Foundation. “And there’s the cost of leaving work to make a second appointment. If this can be done instead of a biopsy, there would be a real cost reduction.”

Up to 1 million biopsies are performed each year on suspicious breast tissue detected by mammograms and self-exams, but as many as eight out of 10 of these biopsies find that the lumps are benign.

Biopsies can cost $200 to $1,000, depending on whether some fluid or an entire lump is removed, and it can take days or weeks to get the results. The cost of elastography is not yet clear, but some experts said the procedure might run $100 to $200. And it can yield results in minutes.

When checked against biopsies of women’s breast tissue, the ultrasound technique correctly identified 17 out of 17 cancerous tumors, and 105 out of 106 harmless lesions. The findings were reported at a national radiology meeting in Chicago this week.

Scientists said the approach may also be used someday to rapidly diagnose damaged hearts and guide the treatment of prostate cancer.

The technique was pioneered during the 1990s at the University of Texas Medical School at Houston by Jonathan Ophir and his colleagues.

Ophir describes elastography as a way to measure and picture the elasticity of body tissue. In effect, it is an extension of one of the oldest tools in medicine, palpation, in which a doctor feels the shape and firmness of body tissue.

To explain elastography, Ophir likens the body to a box-spring mattress, but “a crazy mattress made out of millions of small springs and each one is a little different. Each is moving around at a different rate, depending on their individual stiffness.” Cancerous tumors are like stiff springs. Normal tissue and benign lesions compress more easily.

Both traditional ultrasound and elastography use echoes from high-frequency sound waves to create pictures of what is going on inside the body, but elastography goes a step further.

In traditional ultrasound, a doctor or technician places a handheld device on the skin that sends high-frequency sound waves into the body. Organs and tissue reflect the sound back as echoes, which are sent to a computer that turns them into a picture. Many people have seen ultrasound images of fetuses in the womb.

Elastography, though, also gauges movement. As the doctor moves the handheld device against the breast, the device collects echoes before and after the compression or movement of the breast tissue. The resulting images show stiff tissues as dark areas and soft tissues as light areas.

Breast cancer shows up larger on an elastogram than it does on a traditional ultrasound image, perhaps because the elastogram can “see” the scar tissue around the cancer, Ophir said.

“It’s like finding a marble in Jell-O,” said Dr. Richard Barr, a professor of radiology at Northeastern Ohio Universities College of Medicine who reported his findings at the Radiological Society of North America annual meeting. Germany-based Siemens AG provided the ultrasound equipment and software for Barr’s study.

Ophir and other researchers said breast cancer diagnosis will be elastography’s first real-world application.

“If it doesn’t fly there, it won’t fly anywhere,” said Elisa Konofagou of Columbia University, who is testing elastography on animals and humans to determine the extent of damage after a heart attack. Uses in prostate cancer and thyroid cancer also are under study elsewhere.

Dr. Constantine Godellas, a cancer surgeon at Rush University Medical Center, said some patients and doctors would have trouble giving up biopsies, even if further research confirmed elastography’s accuracy. Doctors may fear lawsuits if they do not order biopsies, he said.

“With the medical legal climate the way it is, that’s a tough call to make,” Godellas said. “It won’t be until a lot more research has been done that people will really buy into it.”

Dr. Ellen Mendelson, chief of breast imaging at Northwestern Memorial Hospital in Chicago, predicted the technique will be used, but may not supplant biopsies, which are becoming less invasive.

“The goal of reducing unnecessary biopsies is laudable, but you don’t want to miss a cancer,” Mendelson said.

By now most of you will know that colds and flu are going around.

This year the symptoms seem to be more protracted ["memushag"] then in other years.

I wrote you in the past already about tea’s for fever/flu, and recently also about a cough formula [primula] and a few days ago about osciloccocinum as prevention.

Today I would like to write you about a formula I am using already many years to strengthen the immune system [if you want to call it this way].

There is all kind of variations possible but for the sake of clarity I will write one formula that virtually everybody can use.

If you are treated with very exceptional drugs like coumadine or with a lot of other drugs then first discuss the use of this herbal formula with a doctor who deals with herbs.

The formula can be bought in homeopathic pharmacies, I advise to buy them in the pharmacies I am working with, because they keep the various compounds in stock [French pharmacy in Gilo or Oplatka on rehov jafo 110].

You may print the following out and show it to them, if you wish to buy the drops:

Astragalus 20cc / Echinacea 30cc / Tropaeolum majus 30cc / Cetraria isl. 10cc / Myrrha 5cc / Usnea 15cc, all of these compounds as a mother tincture [T.M] and all together in one glass bottle [not plastic!!].

Ask the pharmacy to give only the tinctures made by ketzar כצר ,as these are of a good quality.

How much to give?

I will give you some rough estimation;

6 months to one year: 5 to 8 drops

1-3 year: 8 to10drops

3-7 years: 10 to 15 drops

7-12 years: 15-20 drops

12-18 years: 20-30 drops

Older: 30-40 drops

This is a rough estimation; giving some more is not dangerous.

It may be good to start with half of the amount prescribed for one or two days and if tolerated well to give then the amounts as prescribed above.

When and how often to give?

To any child or adult who is often sick [presuming that he get good, nutritious food: how often should we repeat this!!?] one can give these drops for a 2 weeks in a row and then interrupt for 4 days or so and then start again and repeat this cycle.

The drops may be given 3 times a day [or in acute diseases even up to 5 times] and after a 2 to 3 weeks one can give them twice a day for another 3 weeks and then only once a day[for example during the whole winter, or even in summer, if necessary]

The drops can be given with some fruit juice [or grape juice] or honey water.

They may be given before or after food.

There are many other combinations possible, but I have good experience with a combination like this for many years.

As time is running fast forward: the drops from ketzar are made with alcohol which is kosher for pesach and may be given on pesach.

They keep well over many years and don’t throw them out, because of a “cleaning-instinct” [don't waste money]

Have a healthy winter with or without these drops!

Cancer patient Renee Relin says, how did they figure that you go from a sea squirt to a drug that s going to work on this specific, esoteric kind of cancer? She was diagnosed seven years ago with soft tissue sarcoma — a cancer that affects muscles and organs. When doctors discovered a third tumor, she volunteered for a clinical trial testing ET-743, a drug made from the toxins of a sea squirt.
When asked about the study s success rate, Harvard Medical School oncologist Bruce Chabner, M.D., says, about 10 percent of the patients respond, that is, the tumors shrink significantly. For Relin, it offered hope when there was none.
The Israeli yellow scorpion is the inspiration for another cancer drug, TM-601. This synthetic copy of the scorpions venom is helping patients with brain tumors.
There are a variety of compounds in nature which are known to specifically excite cells in very detailed ways, some of them being toxins that come from insects, snakes, things like that, says neurosurgeon Adam Mamelak, M.D., of City of Hope Medical Center in Duarte, Calif.
A Brazilian pit viper s venom is used to make captopril, an FDA-approved drug that lowers blood pressure. ABT 594 is derived from the poisonous skin of a South American frog. It blocks pain 200-times better than morphine and is not addictive.
The poisonous cone snail also helps people with chronic pain. SNX-111 is a very promising drug. It has shown efficacy in patients where nothing else has worked, says Edgar Ross, M.D., an anesthesiologist at Brigham and Womens Hospital Pain Clinic in Boston.
Pharmacist Paul Doering, of University of Florida College of Pharmacy in Gainesville, advises patients considering venom therapy to proceed with extreme caution. He says, If we are talking about snake venom, the margin of safety may be quite narrow, and you may be thinking you are given a therapeutic dose, but it may turn out to be a toxic or deadly dose. It is a matter of balancing the benefits against the risks.
Doering also worries that long-term venom treatments could trigger life-threatening allergies. They are foreign substances to the human body, and the human body s goal is to recognize them as foreign and get them the heck out of the body.
Dr. Chabner, who studies the sea squirt drug, is more optimistic about the future of these therapies. He says, these marine organisms are very, very prolific and a good source of toxins for investigation. There are still huge numbers that could be collected and tested
Relin hopes scientists continue looking under the sea. Thank God for whoever is got the patience to keep trying on stuff, but I think it s phenomenal, she says. Since her first treatment, she has been cancer-free, and credits the sea squirt with giving her extra time with her two sons.
Doctors say the drug from the sea squirt looks most promising for ovarian cancer. This past summer, the FDA granted fast track status to TM-601, the scorpion-derived brain cancer drug. Fast track is designed to speed up the approval process and get breakthrough drugs to the market quickly.

Therefore if one drinks wine one on erev Pesach he should drink a larger [not to small] amount] so that he will not suppress his appetite for the seder.
In the journal Science [1982] it was published that an rapid infusion of glucose in the duodenum very much increased the appetite of rabbits and a slow infusion decreased the appetite.
This is may explain the statement of the Talmud.

So for over-eaters: it is clear what to do.
Drink a very little bit of wine before the meal [one or two teaspoons]

[Rambam mishne tora [ chapter 5,halacha 2 ]:

“With regards to these five species of grain: if [flour from these species] is kneaded with fruit juice alone without any water, it will never become leavened.
Even if [flour] is placed in [these] juices the entire day until the dough rises, it is permitted to be eaten [on Pesach], for fruit juice does not cause [dough] to become leavened.
It merely causes [the flour] to decay.
The following are [included in the category] of fruit juice: wine ,milk, honey, olive oil, apple juice, pomegranate juice and all other similar wines ,oils, and beverages.
This applies so long as no water whatsoever is mixed with them.
If any water is mixed with them, they cause [the flour] to become leavened.”

Simply, the smallest amount of added water will cause the mentioned grains to become leavened[to become chametz]
It is fascinating to be able to remark here the halacha brought by the Rambam and to relate this to the fact that in traditional medicine [as well in homeopathy] all water is not just water.
Water in fruits is considered to be totally different
from just plain water [ both of them are H2O in chemistry]
In homeopathy water that is shaken very vigorously has found to be different [from the water before it has been shaken] when examined with atom spectrograpy.
It is as if water has a “memory” [like footprints in the snow, which show-like a memory- that somebody passed there some while ago]

On seder Pesach all of us will remember the leaning on the left side.
[Kitzur shulchan aruch [chapter 71,halacha 5 ]
“It is beneficial to begin one’s sleep while lying on the left side, and afterwards to change to the right side.
This is beneficial to one’s health, since the liver is on one’s right side, and the stomach on one’s left. When one leans to the left, the liver will lie on the stomach and warm it with its heat. This will hasten the digestion of the food. After the food is digested, it is preferable to lean to the right, thus allowing the stomach to rest and the wastes to descend. One should not turn frequently from side to side”
Do you know that I found in only one medical textbook form 1959 that the temperature in the liver veins is above 41 degrees!!
Thus, it becomes understandable that lying on the left side “cooks” the food in the stomach [see above] which means that the speed of digestion is increased significantly!!
Do you know that recently it has been found that people who sleep only one on side have more tendency for kidney stones?

No, not about bird’s flu.
Something far more beautiful and fascinating.
This coming shabbath we are starting reading parshat
shemini [at mincha]
I am now quoting [see for the source at the end of the
article]:

In Nissan ,the skies of Eretz Israel are full of
birds.
Because this tiny strip of land is on the main
migration route to and from Africa ,it is the home to
a disproportionately large number of birds each
spring.
They each know “their appointed time……………the time of
their coming.”

Now, the fascinating fact is that the chasida [the
stork] is mentioned twice in the torah, in parshat
Shemini and in parshat Re’eh.
And pele pela-iem [miraculously] most of the time the
chasida flies over Israel [first to the south and then
back to the north] dafka in the 2 weeks that these
parshiot are read [in the spring and in the fall] in
the synagogues.

Quite a while ago I sent you some writings about the treatment of lice , as I learned about it in a tropical course in London.

I will resend you the article together with an article quoted from the British medical journal, one of my favored journals, which a friend forwarded to me.

Most people get some shivering, if they hear this word.

May be his is still the aftermath [to-tsa-a] from Jewish history and pharaoh [par-o]

Anyhow, i get a lot of questions what to do with lice.

There are endless “home-remedies”, but let’s talk about the most effective, simple method.

Don’t use the commercial anti-louse shampoos etc, they are for sure toxic, especially for small children.

Some facts about the family-life of lice, in order to understand how to treat a “lousy head”.

Lice like to live very near the scalp [karkevet].

They don’t easily jump from one head to the other, they like their host best!!!

If one find living lice [or something that looks like an egg of a louse]

Further then one centimeter from the head [karkevet], then one can be sure that he/she is going to an old age home to die.

That means, this louse will not form any more a family and is going to die sooner or later! The danger has past [sof sakana]

As long as the louse is living on a short-one centimeter distance- from the head, she can still form a huge family.

So to summarize: lice or eggs [or things that look like that] on the hair, further then one centimeter are in principle grave-tombs [kevariem]

What one has to do is treat the nearby lice?

How? [ ke-tsad]

Use a fine metal lice-comb.

Comb the head [just the karkevet, not the whole length of the hair ,this is difficult and a waste of time, for you are then treating dead animals or eggs, even if they still jump around there on some distance],

As I said ,comb the head every day for 2 to 3 weeks for a few minutes and you will harm all the young, fertile lice.

You may “break their legs or arms” by combing and they will not be able to continue to live [to be fertile], that means no new eggs, no new families possible [they are probably no orthopedic doctors between them]

after 2 to 3 weeks the cycle [machzor] is broken [no births of new baby-lice] and if you still find lice jumping around, leave them alone………they are going to die, just as all the eggs on a distance from the head which are already kewariem [graves] ,as I wrote before.

I hope the story is clear.

If not please write me and I will try to clear up unclear passages in the future.

This story I learned many years ago from a world expert on lice during a course in the tropical school of medicine in London and believe me:he seemed to know everything about these little wonderful creatures.
